Recordkeeping Tips For Pesticide Applicators and Technicians For a minimum of three years, keep the following Information as part of your daily application records: Dosage Rate: Enter the amount of pesticide used for a given application, (e.g., 1 fluid ounce/gallon (FL/G); 1 gram/12 inches (GM.IN); etc.) (This is the premix or final mix, dosage rate as listed on the pesticide label.) Method of Application: Method used to apply the pesticide (broadcast, hand placement, aerial, etc.) Target Organism(s): enter the name of the targeted pest(s). The targeted organism must be listed on the pesticide label. Place of Application: Enter where the pesticide was applied (greens 8 & 7 of golf course; front lawn area; restaurant kitchen; etc.) Recordkeeping Responsibilities All required records must be available for the inspections upon request by authorized DEC staff within a reasonable time and must be sufficient to identify the location of the application and the individual responsible for making the application. Broadway, Albany, New York 12233-7256 Reporting Tips for Pesticide Applicators and Technicians A Report must be sent to DEC by February 1 each year, even if NO applications were made. For each pesticide application during the previous calendar year, keep a record of: EPA Registration Number Enter the EPA registration number from the product label. Include dashes as appropriate. Product Name Enter the registered product name, not the active ingredients. Quantity Used Enter the UNDILUTED quantity applied (amount used out of the manufacturer's container.) No fractions allowed, use only decimals. Units Enter the unit of measure applicable to the quantity used. Use only the abbreviations on the DEC instructions. Date Applied Enter the date of the pesticide application in (MM/DD) format: 0101=January 1, 1114= November 14. County Code Enter the county code where the pesticide was applied. (Use the county codes from the DEC instructions.) Address Enter the street address (including street number) where the pesticide was applied. PO Boxes area NOT allowed. Only list one address on each line of the form. Municipality Enter the name of the city or village (not the township, county or borough.) Do not abbreviate. Zip Code Report the five-digit zip code where the pesticide was applied. Zip code should match the city or village listed. Reporting Responsibilities Properly completed, the Applicator /Technician Pesticide Annual Report, Form 44-15-26 provides all the information required by the Pesticide Reporting Law for reporting purposes. The burden of reporting is placed on the certified commercial applicators and technicians, not on registered pesticide businesses. Certified commercial applicators are responsible for reporting all pesticide applications made by apprentices who work for them or under their supervision. Every certified commercial applicator and technician must submit a report by February 1 of each year, even if no applications were made.
